Apply for social assistance, care assistance for residential care outside Hamburg

Are you in need of care, would like to move permanently into a nursing home outside of Hamburg, and are unable to finance the care you require yourself? Learn how to apply for a needs-based social benefit with the Care Assistance Program.

Detailed description

You can apply for care assistance if you are unable to pay for the care you need from your own resources. This may be due to physical, mental, or psychological limitations or health problems that you cannot manage alone.



If you have care levels 2 to 5, you can apply for the following benefits:

  • Permanent inpatient care if the nursing home is located outside Hamburg.
 

Information

Prerequisites

  • You are in need of care within the meaning of Social Security Code XII.
  • You cannot cover the necessary care needs from your own financial resources.
  • If applicable: Your spouse or life partner, who is not separated, cannot be expected to provide the necessary funds from income and assets to cover your care needs.
  • You do not have long-term care insurance or
  • do not receive any benefits from nursing care insurance or
  • the benefits of long-term care insurance are not sufficient to cover your care needs.

Documents required

  • all identification documents (if applicable, for foreigners: with residence permit),
  • Evidence justifying need, such as medical certificate, proof of care level, nursing home contract for full-time care
  • Proof of priority care benefits (nursing care insurance fund)
  • Proof of financial need (social assistance application)
  • If necessary, you will receive further information from the responsible authority.

There are no special instructions to be observed.

Deadlines

You can receive care assistance from the moment you report your need for assistance. Report your need for assistance as soon as possible.


Do not apply for care assistance until you have all the necessary documents.

Procedure

You inform the responsible authority about your need for assistance.
You submit a fully completed social assistance application to the responsible authority.
The responsible body will review your application and supporting documents. If necessary, it will request additional documents or information from you. If necessary, it will obtain further opinions.
The responsible authority will decide on your application.
You will receive a notification.

Processing time

If all required documents are complete, a decision on your application will be made as quickly as possible.
